If you have
dental insurance, we are happy to help you maximize your benefits.
Our office will file your insurance claims for you. Please understand
that dental insurance is intended to cover some, but not all of
the cost of your child's dental care. Most plans require a patient
responsibility, deductible or other expense which must be paid by
the patient. Your insurance is a contract between you, your employer,
and the insurance carrier. You are responsible for all charges to
your account regardless of what your insurance pays. To avoid misunderstandings,
our business staff invites early discussion of financial problems
or questions regarding fees, payment from insurance carriers, or
any other issues.
Payment is expected
at the time of service. Payment can be made by cash, check, MasterCard,
VISA, American Express, or Discover. Failure to pay at each visit
will result in a billing charge unless prior arrangements have been
made with the doctor.
The adult that
brings the child to the appointment is responsible for making the
payment for the services rendered.
Many
patients request that we accept assignment of benefits from
their insurance carrier. This means the insurance check
would come to our office. We will be happy to do this provided
you allow us to charge any amount not covered by insurance
to your MasterCard, VISA, American Express, or Discover
account. We will need a Pre-Authorization
form on file to provide this service.
